Paraoptometric of the Year Award Nomination Form

The Wyoming Paraoptometric Association (WPA) Paraoptometric of the Year recognizes the deserving individual paraoptometric for the performance of outstanding service on behalf of the profession, community, and the Wyoming Paraoptometric Association.

Nominations will be accepted until December 15.

Criteria: 

• Service to the Optometry and Paraoptometric Associations
• Office competency-office management and technical skills.
• Service to Paraoptometric association (state, regional, nation) as an office, committee chairperson, member, etc.
• Contributions of personal time and effort on behalf of the profession’s advancement.
